Yes you will.

Everyone must sign the daily roster so that the John Jack has a list of all divers and crew on the boat for the trip.  Each diver must also sign a waiver / liability release before the boat leaves the dock.  Divers under the age of 18 must have a parent or guardian co-sign the form. 

Every passenger must fill out a Diver Data Sheet on their first trip of the season.

Certain dive trips may require an additional special waiver that must be notarized. You will be informed if this waiver is necessary prior to the trip. This waiver is mandatory for all dive trips to the Andrea Doria.
